Job Description

  1. Administers prescribed medications.
  • Determines medication to be administered, its actions, therapeutic effects, adverse reactions, etc.

  • Administers medications per protocol.

  • Follows the 5Rs of medication administration.

  • Uses knowledge in the administration of medications based on a patient's age.

  1. Participates in the education of patient and/or significant other.
  • Explains procedure and intervention to patient as indicated/requested.

  • Recognizes and utilizes health teaching opportunities and resources/materials available for teaching.

  1. Follows departmental policy and procedure with documentation.
  • Utilizes care maps and progress notes to reflect patient's status.

  • Uses methods of documentation according to departmental policy.

  1. Administers nursing care to assigned patients under the direct supervision of a Registered Nurse.
  • Gives hygienic care to patients assigned and oversees/assists with activities of daily living; performs assigned treatments and procedures.

  • Takes vital signs of patients assigned and reports abnormal values to the RN in charge.

  • Reports observations and unusual conditions of patients to RN.

  • Collaborates with physicians and other health care team members in developing and implementing plan of care.

  • Follows principles of infection control and patient safety.

  • Assists patient to wheelchair or stretcher.

  1. Performs core procedures/treatment including, but not limited to care of patients requiring restraints; CPR; Unit specific procedures/ treatment; phlebotomy; and EKG exams.

  2. Functions effectively within the organizational structure.

  • Accepts assignments and seeks clarification as needed.

  • Carries out assignment responsibly.

  • Proactively reports patient care observations and findings to RN regularly.

  • Participates as an integral part of the nursing team in assuming additional duties and responsibilities.

  • Assists with the admission process as directed.

  1. Performs related duties, as required.

  • High School Diploma or equivalent, required

  • Licensed as a Practical Nurse in the State of New York.
