As a Case Management Coordinator, you will be responsible for behavioral health discharge follow-up and telephonic service coordination in the state of Kentucky.
The Care Management Coordinator's primary responsibility will be completing telephonic discharge follow-up calls for Aetna Better Health of Kentucky members being discharged from behavioral health hospitals and inpatient psychiatric facilities as required under the Medicaid Managed Care Organization contract with the state and outline in HEDIS measures. The position may be required to case manage members identified as supportive service candidates, service coordination including locating behavioral health providers, other providers as appropriate and serve as the MCO representative for state hospital meetings.
2 years experience in behavioral health, social services or appropriate related field equivalent to program focus (Required)LCSW (Required)Behavioral case management and behavioral health discharge planning (Required)Managed Care experience (Preferred)Case management and discharge planning experience (Preferred)Crisis intervention skills (Preferred)Foster Care and pediatric behavioral health experience (Preferred)
The highest level of education desired for candidates in this position is a Master's degree.

Mental Health/Certified-Advanced Social Work Case Manager is required
Managed Care/Managed Healthcare Professional is preferred

Position will be in Office in Louisville, KY.
Telework is based on budget, seniority, must work in the office at least 6-9 months without performance issues. Telework is not guaranteed.
